All enrolled students at the University of Tennessee Health Science
Center must have health insurance. Gerald Holland Insurance,
Inc. is currently the agency for student health insurance coverage.
University of Tennessee Health Science Center in partnership with Aenta Student Health, an Aetna company, is excited to announce the availability of the 2008-2009 Student Health Insurance Plan. We offer a student-focused health insurance plan that protects you at school, at home and while traveling or studying abroad. What Is The Health Insurance Plan All About? The following are highlights of The Injury and Sickness Plan coverage; more detailed information can be obtained in the Plan Brochure.
• $150,000 aggregate maximum benefit per accident and sickness
• $250 deductible per insured person per policy year
• Access to a national network of preferred providers
• 80% coverage for services rendered with Aetna’s preferred providers
• Prescriptions covered with $5 Generic and $10 Brand Name copays up to a maximum of $2,000 per year
• Discounts on Value-Added services such as Vision and Alternative Health
Why Is A Student Health Insurance Plan Important? The University will not assume any financial responsibility for your medical bills, and you are strongly encouraged to maintain health insurance. If you have been covered by a parents' policy, please review it carefully to make certain you are still eligible for coverage. Employer plans often do not cover students after they reach a certain age, and HMO's may provide limited coverage when away from home. If you are not adequately covered, be sure to take advantage of the University of Tennessee Health Science Center Student Injury and Sickness Plan by enrolling . Got Questions? To All Students Enrolling in the UT Student Health Insurance
1. If you are purchasing the new student health insurance for YOURSELF ONLY, you do not have to do anything except pay your fees. The ID card will be mailed to you. EXCEPT, if you did not have the UT insurance last semester, you will need to fill out an application for yourself and pay the fee.
2. If you want to cover YOUR DEPENDENTS, you will need to fill out an application for them and pay the extra fee. Then an ID card will be issued for them.
The application forms are available at University Health Services, Suite 922, 910 Madison Bldg., and in the Office of Student Life and Student Affairs, in SAC - Student Alumni Center. They are also available on the Aetna Student Health website at: www.aetnastudenthealth.com.

08/09 Student Dependents Insurance fees and Enrollment Form
You have the option of
maintaining your personal health insurance policy. If you wish to be covered
by your personal health insurance policy, you must fill out the insurance
verification form (see below), attach a copy of your health insurance card
and return it to the UTHSC Office of Student Affairs.
This form must be completed and submitted with insurance information
every semester of enrollment at UTHSC.
THE INSURANCE
VERIFICATION FORM AND INSURANCE CARD MUST BE SUBMITTED BEFORE YOU REGISTER. If
you did not submit your insurance verification form and a copy of your
insurance card to the Office of Student Affairs before you registered,
you will be automatically charged the cost of the student- only coverage
of the UTHSC student health insurance on your fee sheet.
If you have registered
and want to maintain your personal health insurance, you must submit the insurance
verification form and your insurance card to student affairs and contact Gerald
Holland Insurance, Inc. at 662-895-5528 for a refund of your student health
insurance premium within one week of registration.
Please direct all questions and claims inquiries to Gerald Holland. He can
also help you with questions regarding the terms and conditions of the UTHSC
student health insurance policy.
