Immunization Form for 2008
Dear Prospective UT Student:
Attached is a gold immunization form that must be completed prior to your enrollment at the University of Tennessee Health Science Center. The Infection Control Committee of UTHSC requires that students be immunized and tested for communicable and vaccine preventable diseases (see back of Immunization Form).
All entering students for Summer or Fall, 2008 must complete this form and mail or fax to the address above by June 4, 2008. The form must be complete and have your name, college, social security number, and date of birth on it to give credit for your information. All immunizations, with the exception of Hepatitis B, must be up to date for enrollment at UTHSC and proof of immunizations or immunity is required.
TB skin tests do not need to be complete before admission. TB skin tests will be given during registration as many agencies require this test before clinical rotation.
The Hepatitis B immunization series and positive titer are required of all students and you have one academic year to complete this requirement. You can begin this series during your first semester of enrollment at University Health Services. You have until December 5, 2008 to start the series of 3 injections. If you have completed the Hepatitis B series, you must provide proof of immunity by titer. You will be charged $200 on your fee sheet for the series of 3 shots and a titer, UNLESS you show proof of all 3 components to UHS.
