Obtaining Services & Necessary Provisionary Forms

It is the responsibility of the student to contact SASS in order to make an
appointment for an accommodation consultation prior to matriculation or as soon as the need for an accommodation is identified. An appointment can be made by calling the Disability Coordinator at 448-1452 or emailing lbrook15@utmem.edu.

Provisionary Forms

Students will need to complete all provisionary forms and submit documentation before accommodations can be made. Forms are also available in the SASS office. Once complete, submit forms to the SASS office in person or via fax at 901-448-1451.

Documentation Guidelines

To provide reasonable and appropriate academic accommodations to those students with disabilities at UTHSC, SASS requires documentation that shows the current disability and the impact it has on academic functioning.  The documentation must adhere to the following guidelines to ensure it is appropriate for UTHSC.  The documentation guidelines can be found in the Student Disability Handbook.

Documentation of the disability must be provided within two weeks of disclosure of the disability. During this time, students will be given “temporary concessions” if needed. The time for “temporary concessions” will not be extended beyond two weeks. Documentation can be faxed to 901-448-1451, but a hard copy of the documentation must also be received. Guidelines for acceptable documentation are available in the SASS office and on the SASS website.

Upon receiving the documentation, the Disability Coordinator will send the documentation to the independent disability consultant.  After review of documentation, a written decision regarding the provision of accommodations will be emailed to the student.

Testing Accommodations

Students who are granted testing accommodations are required to submit a copy of the class schedule and testing dates prior to the end of the second week of each term.

Students requesting accommodations other than for testing may be asked to participate in a meeting to determine the most appropriate course of action for a course or clerkship.

Students who are denied accommodations have a right to appeal the decision.
