The UTHSC IRB is no longer accepting paper applications. Beginning September 2008, the UTHSC IRB began using iMedRIS, a web-based system that allows investigators to submit applications electronically. Click on the iMedRIS tab on the left to access the iMedRIS system to submit your application.
New Study Applications:
Beginning September 2008, all new studies must be submitted electronically via iMedRIS.
Continuing Review:
For studies initially approved by the IRB on paper, you will need to convert your paper study to an iMedRIS study prior to the submission of the renewal application in iMedRIS. See the instructions for converting a paper study to an iMedRIS study (Registration of a study in iMedRIS) or contact the IRB office for assistance.
Form 2 (revision)
Application for revisions of or changes in research protocol and/or informed consent form. This form may only be used for studies that were initially approved on paper and a revision needs to be reviewed prior to the registration of a study in iMedRIS. For example, you may submit a paper application for revision if the key study personnel need to be revised so that the study may be successfully registered in iMedRIS. For all studies previously approved in iMedRIS, all associated amendments and other changes to the research must be submitted electronically using iMedRIS.
Form 4 (adverse events and unanticipated problems)
Report an adverse event (local or external) or an unanticipated problem. This form may only be used for studies that were initially approved on paper and may be close to terminating or have been terminated. For studies previously approved in iMedRIS, all adverse events and unanticipated problems must be submitted electronically using iMedRIS.
Form 7 (closure)
Report of termination of IRB project. This form may only be used for studies previously approved on paper and were not registered in iMedRIS. For studies previously approved in iMedRIS, the report of termination must be submitted electronically using iMedRIS.
