Steps to Submit rDNA Registrations


STEP 1. Complete the rDNA Registration Form.
Download the “rDNA registration form” (Recombinant DNA Registration Form) using the link in the left sidebar or by going to http://www.utmem.edu/research/research_compliance/IBC/docs/UT_rDNA_AUG2009_Final.doc. Follow the instructions on the form or use the more detailed instructions provided in the “Instructions to complete rDNA Registration Form” link in the left sidebar, or by going to http://www.utmem.edu/research/research_compliance/IBC/docs/

. Type directly on this form using any word processing program that is compatible with Microsoft Word.


STEP 2. Sign and Date Registration (required signatures include the Principal Investigator and the Department Head). Note: You may send an unsigned version for pre-review, but only signed registrations will be reviewed and considered for approval by the IBC at its regularly scheduled meetings.


STEP 3. Submit the Registration to the IBC office.
You may use either of the following two methods for submission of your completed and signed Registration:


(1) Forward the original signed registration via campus mail to: Institutional Biosafety Committee, 910 Madison Avenue, Ste. 650.


(2) Scan and email the original signed registration as a pdf document. Keep in mind that the pdf file must include the principal investigator and the department chair's signatures with dates, and should be of sufficient quality for IBC members to read. Email the pdf to the UTHSC IBC office at ibc@utmem.edu and send the original signed copy to the IBC office by campus mail.


The IBC meets on the first Friday of each month. If you wish to have your registration reviewed at the next scheduled meeting, the registration with all administrative signatures should be received in the IBC office no later than 12:00 noon on the 3rd Friday of the prior month.


Note: We strongly suggest that you submit your registration early for pre-review and give the IBC office a chance to assist you in making changes prior to full committee review. This process will expedite the approval of your registration.
