Arrange Pickup of Surplus Property

The following steps should be taken to arrange pickup of surplus property for departments of the University of Tennessee Health Science Center:

  1. Fill out and complete the UTHSC campus Surplus Property Transaction Form. All applicable fields, except for the Account No. should be completed including Condition Code(s) and Asset Numbers. The department head or designated administrative officer signs off on the Approved by Current Department Owner line.
  2. Completed forms can be faxed to Surplus at 448-2960, sent via campus mail to 822 Beale-Room 141, or scanned and sent electronically to surplus1@uthsc.edu. If the form is faxed or sent electronically, please do not send a follow-up copy. Surplus reviews all forms before we forward them to the Facilities dispatcher to schedule a pickup by Facilities Logistics personnel. Surplus may make alternate arrangements for surplus property located away from the main campus in Memphis.
  3. Facilities Logistics personnel should present a copy of the Surplus Property Transaction Form to the department upon arrival for pickup.
  4. Department personnel should verify that all items on the Surplus Property Transaction Form were picked up by Facilities Logistics personnel and Facilities Logistics should sign and date the form.
  5. Surplus personnel takes care, custody and control of the surplus property upon receipt into Surplus; the form(s) are signed and a copy is faxed to the department head or designated administrative officer that originally initiated the pickup of the surplus from the department. The department should retain the signed copy of the form in accordance with University Fiscal Policy.
  6. Surplus forwards copies of all signed Surplus Property Transaction Form(s) to UTHSC Accounts Payable on a weekly basis where they are reviewed. They identify any equipment that is on inventory that can be deleted from inventory records, and then they complete the T-64 and attach a copy of the signed Surplus Property Transaction Form. All completed documentation is forwarded to Knoxville.
