Obesity Management Resources

OBESITY RESOURCE MANUAL

Dear  Primary Care Physician/Provider:

In order to fight childhood obesity and its associated conditions, we have developed a toolkit for the prevention, evaluation and treatment of children who are overweight or at risk of overweight.  You and your staff can use this toolkit to implement an effective weight control program.  We assure the parents of overweight children that if they adhere to the Rearing Healthy Children Guidelines, they will be successful in improving the health of their children and other family members.  To date, we have been encouraged by the results of using these guidelines in our lifestyle patients.  Long-term follow-up is in progress.

Pages:

1-3    Rearing Healthy Children:  Guidelines for Children Who are Overweight.  The most effective way we have found to use these guidelines is for the physician or nurse to read them to the parents and answer the questions.
4-7    Healthy Lifestyle Initial Evaluation (Patient) - to be completed by parents
8       Healthy Lifestyle Initial Evaluation (MD) - to be completed by the physician
9-11  Healthy Lifestyle Return Visit (Patient) - to be completed by the parents
12     Healthy Lifestyle Return Visit (MD) - to be completed by the physician
13     Healthy Lifestyle Program Laboratory Test Results
14     Healthy Lifestyle Program Follow-up Flow Sheet
15     A copy of a sample food and exercise log (diary)
16-18 Exchange list of vegetables and fruits; dinner plate
19     ICD9 Codes for Diagnoses and Billing

These guidelines are an attempt to coordinate our resources for the benefit of overweight children.  We feel that the primary care provider plays an important role in reversing the epidemic of obesity in children.  We anticipate that you will be successful in managing a large percentage of the overweight children in your practice.  Children who are resistant to lifestyle change can be referred to a center specializing in weight control in children.
