Application Checklist and Tips
Applicant Checklist:
Two part application process:
- Web-based component - submitted online
- Paper-based component - submitted as one packet
Web-based component:
- Online application
- Online $50, non-refundable application fee
- Essay must be submitted online, as part online application
Paper-based component:
- Applicant is responsible for collecting all paper application materials that cannot be submitted online (e.g. transcripts, Recommendation Forms, licenses, certifications, etc.)
- Materials include:
- Official copies of all college/university transcripts
- Three (3) recommendation forms
- Copy of a Basic Cardio Life Support (BCLS), Advanced Cardio Life Support (ACLS), PALS and maintain throughout the program.
- Copy of all nursing licenses and certifications
- Applicant is responsible for submitting all paper application materials directly to the UTHSC Office of Enrollment Services.
- Applicant is responsible for submitting all materials in one packet, unless material is unavailable (e.g., final transcript not yet available).
