Please return your registration form with remittance by February 19, 2010. (Please note — we encourage you to register early and take advantage of the discounted fee for registration by February 19, 2010.  Advance registration for all programs is strongly encouraged and appreciated.) If the dentist from your office is not registering, the first staff member must register at the dentist’s rate.  Check-in for the Dental Congress begins Thursday afternoon at the Hilton Hotel and will continue all day Friday, Saturday and Sunday mornings.  All information, nametags, and meal tickets will be held for pick-up at the registration desk.  Your registration fee covers your choice of sessions, Friday and Saturday lunch, refreshment breaks, continuing education credit, admission to the Welcome Party, and exhibit booths.

Cancellation Policy
In order to ensure adequate arrangements for courses and other events, no refunds will be given after February 26, 2010. Please submit a written request for refund postmarked or faxed by that date. A $30 processing fee will be charged for each cancellation processed. Refund checks will be mailed within three weeks of adjournment of the MidSouth Dental Congress.
