Health Insurance

Health insurance becomes effective the 1^st day of the month following one full calendar month of employment. State Group Health Insurance also includes basic term life and basic accidental death insurance.

Initial enrollment

Initial enrollment must be made within the first 30 days of employment. Late enrollment is subject to approval and requires a statement of health and a non-refundable late applicant fee. Late applicants are NOT guaranteed coverage.

New employees will complete insurance forms during new hire orientation.

After initial enrollment, changes may only be made:

• within 60 days of a special qualifying event.
• during the "Annual Transfer Period" (Oct. 15 - Nov. 15)
  (NOTE: if you did not enroll during your initial enrollment period, you will not be able to enroll later during the Annual Transfer Period)
