Optional Life Insurance

You are eligible for optional life insurance coverage the day after you have completed three full calendar months of employment.

Initial enrollment

Your enrollment forms must be received by Insurance BEFORE your personal enrollment deadline (same as your deadline for health and dental enrollment).

After initial enrollment, changes may only be made:

• within 60 days of a special qualifying event
• during the "Annual Transfer Period" (Oct. 15 - Nov. 15)*
  *A Proof of Good Health form will need to accompany an application for Optional Life Insurance if submitted during this time

Premiums are based on age and amount of coverage.  Contact the benefits office for assistance.
