Long-Term Disability Insurance

Eligibility:

You are eligible for long term disability insurance coverage the day after you have completed one full calendar month of employment.

Initial enrollment

In order to enroll without a "Statement of Health" from your physician, your enrollment forms must be received by insurance BEFORE your personal enrollment deadline. After this deadline you may enroll at any time, but you will be required to submit a "Statement of Health."

After initial enrollment

• application for changes may be made at any time with the submission of a Statement of Health (coverage for late applicants is not guaranteed)
