Insurance Introduction
Eligibility
Regular employees scheduled to work a minimum of 30 hours (75 percent
appointment) per week are eligible to enroll in the health insurance plans. Enrollment
is not automatic. The employee must complete the State of Tennessee
Insurance enrollment form within 30 days of the date of employment. If the
employee completes all applicable forms, coverage will be effective on the
first day of the calendar month following the completion of one month's
active employment (provided the employee is actively at work).
After initial enrollment you may add additional
dependents to health insurance only within 60 days of a special qualifying
event. Supporting documentation will be required.
It is your responsibility to keep your name, address and beneficiary
information current and to terminate coverage on dependents who become
ineligible for coverage.
