Memphis Aligning Forces for Quality (AF4Q) Regional Quality Strategy. The goal of this grant is to expand the Memphis AF4Q initiative to involve hospitals and nurses in a community-wide quality improvement effort, concentrating on improving care for ambulatory care-sensitive chronic diseases, care coordination, patient-centered care and care for minority populations. The grant supports the Healthy Memphis Data Center led by Dr. Bailey in developing a data driven regional quality strategy and action plan for Memphis and in public reporting of health care quality and disparities information.

Chartered Value Exchange (CVE) designation awarded to Healthy Memphis Common Table. The CVE award supports the Healthy Memphis Data Center led by Dr. Bailey by providing Medicare performance data gauging how well physicians treat patients, technical assistance, information exchange, participation in national meetings, research tools, access to experts, and an ongoing private web-based knowledge management system. The award supports regional reporting of health care quality, price and efficiency information and consumer education regarding quality and value in health care.

Subcontract, "Tennessee Patient Data Sharing Plan Evaluation." This grant is for evaluation of the impact of the Midsouth Health Alliance Data Exchange on disease specific outcomes. The research will determine whether the Data Exchange has reduced unnecessary duplicate testing and hospitalization and overall healthcare costs for emergency evaluation and treatment of chest, back and abdominal pain and headache.

Memphis Aligning Forces for Quality (AF4Q) Regional Market Pilot Initiative. The goal of this grant is to engage consumers and providers in efforts to improve quality of ambulatory care in the Memphis region. This initiative seeks to: 1) help individuals take more control of their own health and healthcare, 2) develop a sustainable infrastructure to support and improve physician care management processes, and prepare physicians for public reporting and pay-for-performance, and 3) create a reporting system that guides individual physician, practice and community-wide quality improvement efforts and helps consumers select and work in a closer partnership with their physicians. Healthy Memphis Data Center
The goal of this grant is to report diabetes and diabetes quality of care trends for geographic areas in the Shelby County area in order to support health and healthcare improvement initiatives throughout the Mid-South by using hard data to track the progress in achieving measurable health goals.

TennCare Disenrollment Impact Study. The objectives of this study are to determine: 1) the ability of TennCare disenrollees to obtain other health insurance - degree to which the "crowd out" is reversed, and 2) the short-term impact of the loss of TennCare coverage on use of the safety net system, access to health care services, health status, out of pocket healthcare costs, personal and family finances, and economic productivity.

Obesity and Diabetes Initiative Data and Evaluation Support Grant
The goal of this grant is to provide scientific evidence regarding best practices and models for collaborative efforts to reverse the obesity epidemic in the mid-south, to assist in establishing measurable outcomes and data collection strategies for the Initiative, and to facilitate collecting baseline benchmark data, establishing intervention outcome goals, monitoring progress toward goals, and documenting outcomes of interventions.
