Preparing a Security Incident Report
UTHSC's Security Incident Reporting System allows completion of required reports through a web-based application accessable at : http://oracle.utmem.edu/w160_log_login.php
The Reporting System contains three screens:
- First, the Login Screen. It requires a NetID and password grants access to the Incident Response Reporting System. Click Here to see a sample Login Screen.
- Next, the Main Menu. Here a new report can be started or an existing report updated or submitted. Click Here for Main Menu Instructions.
- Then, theUTHSC Security Incident Report . The report has two parts: Part One records contact information. It is accessable only from the Main Menu. Part Two records details of the system and incident and is accessable only from Part One. Note that Part One may be completed now, and Part Two at a future time.
