A transcript of work completed by
a student at UTHSC will be furnished without charge within
approximately three working days after receipt of a written,
signed request addressed to the Registrar, provided the
student is not indebted to the University. Processing of
transcript requests at the end of a semester may take
longer.
ENROLLMENT CERTIFICATION
Verification that a student is or
was enrolled at UTHSC will be furnished without charge within
approximately three working days after receipt of a written,
signed request addressed to the Registrar. Enrollment
verifications will be faxed upon request.
Verification
of Enrollment requests are processed through the Office of the

SECURITY CHANGE FORM
To change your name you
must present the original marriage license or divorce
decree indicating the desired name, along with the
change of name form. To change your social
security number you must have the original social
security card or if you send a copy of your social
security card it must be notarized, along with the
change of social security form. ADDRESS

This form
is to be filled out if you're wanting to change your
Student's local address or your permanent address.
Please note that refund checks are mailed to the
Student's local address if available, if not, then
refund checks are mailed to the Student's permanent
address. VETERANS
CERTIFICATION |
Services for Veterans are
provided through the Office of the Registrar. Assistance is
given to veterans and dependents of eligible veterans to
enable them to derive the maximum benefit from their veterans'
educational entitlement.
