HOW DO I APPLY?

 

Prospective students may join the certificate programs of the Tennessee Consortium by applying to any one of the three campuses of the Consortium (East Tennessee State University, University of Tennessee Health Science Center, and University of Tennessee).  Each campus will have its own set of procedures for requirements.  To be eligible for admission, an applicant must meet all the requirements of that campus as well as the overall requirements of the certificate program through the Consortium.

To apply to the Tennessee Public Health Workforce Development Consortium:

  1. Students must be approved for participation by their immediate supervisor and Regional or Metro Director.  Each Regional or Metro Director will need to recommend participants to the Department of Health's Central Office for final approval.  Names should be submitted to Doug Gilbert at Doug.Gilbert@state.tn.us.
  2. Students should be approved by the Department of Health's Central Office before they start the application process.
  3. Students must have a minimum undergraduate grade point average (G.P.A) of 2.7 on a 4.0 scale.  Higher levels may be required on some campuses.

  4. Students must complete the Tennessee Public Health Workforce Development Consortium's application form.

  5. Students must complete the Student Acknowledgement form.

  6. An official undergraduate transcript of each college or university attended to the campus of your choice as specified below.

  7. Full admission status as a nondegree student through the institution of choice (East Tennessee State University, University of Tennessee Health Science Center in Memphis, or University of Tennessee, Knoxville).

To apply to a participating institution:

  1. East Tennessee State University:

    Students will apply to the School of Graduate Studies full seeking graduate degree student.  Requirements include:

    1. A completed application form:   http://www.etsu.edu/gradstud/forms/admission_form.asp;  
      On the top of the application form, write "Tennessee Public Health Workforce Consortium".

    2. An official transcript of each college or university attended;

    3. An undergraduate grade point average of 2.75 on a 4.0 scale;

    4. A brief essay;
    5. Send application materials to:

  2. The University of Tennessee Health Science Center (Memphis):

    Students will apply to the College of Graduate Health Sciences as a non-degree student.  Requirements include:

    1. A completed on-line application form:  http://www.utmem.edu/TNConsortium/UT-Memphis Consortium Application.htm;

    2. Submission of an official transcript from each college or university attended; and

    3. A cumulative undergraduate grade point average (G.P.A) of 3.0 or higher on a 4.0 scale; and  

    4. Send application materials to:

  1. University of Tennessee (Knoxville):
