By answering "Yes" to all of the following questions will ensure you have completed the application process for admission to the Consortium.
- Are you an employee of the Tennessee Department of Health?
- Have you been approved by the Tennessee Department of Health to participate in the Consortium?

- Have you completed the Student Acknowledgement form? http://www.utmem.edu/TNConsortium/StudentAcknowledgementForm.htm
- Have you applied to one of the participating institutions?
